2025 Recognition Student Information Form

The 5th Grade Recognition Committee is ordering 2025 class shirts and every 5th grade student will receive one!
We want your child to have the right size and we want to use their preferred name.

Note to parents: Later this spring we will collect a fee through Membership Toolkit for our Recognition Celebration, which will cover the ceremony, reception, t-shirt, and field trip. Scholarships will be available.
